a pop up then appears into which the notes are typed in:-
There's no set field by name script shown in your screen shot.
The first parameter of SEt field by name has to calculate to return text that exactly names a field with this format:
So the most common failure is for the calculation to not correctly evaluate to the exact TableOccurrenceName::FieldName needed.
A less common error is for set field by name to be set up correctly but in a script that attempts to execute it in the wrong context. In other words when the current layout references the wrong Table Occurrence for the TableOccurrenceName part of the reference to be a workable field reference.
and then pressing submit invokes the following script - and the set field by name line does not populate with the required result. any idea why ?

Just guessing, is it possible there might be a typo. In your set field by name step, the part for notes " notes ", has
an extra space after notes, doesn't appear there is a space in your field list
