Database Unresponsive, Console Shows Multiple Logins for Same Users

Description of the issue
Users begin to complain that the database is unresponsive, particularly when they go from one table to another. When we check the admin console, we can see that users have multiple logins, because they have exited and then tried to log in again, only to find that the database is still unresponsive. We have to reboot the server. We have done file recoveries, to no avail. It used to happen only once in a great while, and we were hopeful that upgrading to v12 would solve it. However, it seems to have gotten worse. Happened with one database twice yesterday, and a different one, on a different server, today.

We expect the databases to run smoothly and respond promptly.
They do not, and users get frustrated, logging out and back in multiple times.

No error messages, just blank screens or hourglasses
Rebooting the physical server is the only "fix" we have found. It doesn't always last long.
