Description of the issue

Admin console lists each available plug-in twice. Event log shows plug-ins failing to load.

I attempted to to enable all visible plugins (including dupes). Event log showed the first attempt for each plug-in failing, then second attempt enabling the plug-in. Within a few minutes thereafter the server crashed.

Removed all plug-ins. Rebooted. Admin console showed no plug-ins. Put plug-ins back. Rebooted. Admin console showed two of each plugin with the log showing the failure to load.

Workaround

None. Currently have plug-ins disabled pending a solution.
