These type of issues are difficult to determine the cause. I would first uninstall and reinstall FileMaker Server 9, and then see if the problem persists.
I looked on our open source files and saw that the latest release of OmniORB is 4.1.3, but I doubt that is the problem. In any case, I would download the latest release.
(click on "Open source files" and select OmniORB4.1 Windows version).

I have reinstalled the server software and it hasn't helped. I am running 3 different servers and FileMaker server is intermittently quitting on all three. All server are running version 220.127.116.116. The only information I'm getting is "FileMaker Server encountered a problem and needed to quit" and "Faulting application fmserver.exe, version 18.104.22.1686, faulting module OmniORB4.dll, version 22.214.171.124, fault address 0x00421e5. Any idea where I can get any other information on why it keeps turning off? I have 800 users who are getting really unhappy!
