FM10 - PHP Site Assistant - Two Machine Deployment - PHP issue

PHP is throwing an error when viewing sites generated by the PHP Site Assistant in a  Two Machine Deployment config.
 
FM Server 10 Advanced installed in a Two Machine Deployment across two OSX Servers running 10.5.6.  Databases serve fine and all the XLST and PHP connection tests to the FMServer_Sample work perfectly! 

 

However any new site generated for the FMServer_Sample database by the PHP Site Assistant gives the following response:

Warning: require_once(Date.php) [function.require-once]: failed to open stream: No such file or directory in /Library/WebServer/Documents/php/fmview.php on line 12

Fatal error: require_once() [function.require]: Failed opening required 'Date.php' (include_path='/usr/lib/php') in /Library/WebServer/Documents/php/fmview.php on line 12
