Validate Field to Prevent Duplicate Records with Same 2 Foreign Keys

I have a table with two fields, both foreign keys that relate Client ID to Insurance ID. Functionally it relates clients to particular insurance plans and allows for the entry of their plan #s and all of that in other fields within that table.

I would like to prevent multiple entries of the same combination of Client ID & Insurance ID. For example, I wouldn't want a user to assign ACME Basic Insurance twice to the same client. Validating the Insurance ID field seems like the logical thing. However, I'm not sure how to construct a calculation to validate that the same combination of Client ID & Insurance ID have not already occurred in this table.

I'd appreciate any ideas.

Thanks so much,
