I did find a previous post by you Using custom database icons that display in WebDirect Launch Center similar to this. You are on a Two-Machine Deployment. Are the following still true?
We are using FMS 18.104.22.1683 on a Windows server (2012R2)
SSL is enabled and we are using a Comodo Elite SSL Certificate.
On a two-machine deployment you would need a certificate per machine.
Also, still using .jpg and .png files?
Do the file names (for the images) contain any non-alphanumeric characters?

What flavor of Windows Server 2012 R2? (Standard, Enterprise, etc.)
Does the issue occur when using the IP Address rather than the domain? (https://IPAddress/fmi/webd)
Do the custom icons show properly in FileMaker Pro/Go?
Does the issue persist if you disable SSL?
Icons fail to show if .jpg or if .png or if I select one of the choices provided by the FMPA such as "Art" the first one that is displayed under the Custom choice.
We have successfully installed the SSL Cert on both servers. However, on the master box, it displays a warning that the Cert is not from an authorized vendor even though InCommon is listed on FMS documentation. I called support about that, and was told this is a known issue.

Re: if I disable SSL- not yet tested. Do I just need to disable in the Amin Console or do something on the worker box too?
We do have a known issue where the error falsely pops up, but it could still be true (unlikely if it's connecting with a green lock).

Correct, just disable via Admin Console and Restart FileMaker Server.
I sent an email to the server admin to check on which version of Server is running. When they built the VM's I remember telling them it had to match the FMS requirements. Awaiting a confirm from them
