FM16A on Windows7 (both 64bit)
When I close it and re-push "Modify..." button, it repeats the issue but not all the time. Some time it shows names.
The file is converted from CSV, have 68 fields.
I'm using FMP Advanced 16 32-bit on Windows 10 right now, but I have seen this issue for as long as I've been working regularly in Windows (around version 12). Sometimes adding/removing fields or checking/unchecking boxes makes the field names appear (or disappear), sometimes exiting and returning changes the display, sometimes the field names will disappear in the middle of scrolling through the field list. FMPro 16 fixed a lot of long-standing Windows issues, but this one is still around.

This issue generally occurs in a shared environment if Table View has been modified by more than one client. That is, if you go into Modify Table View, save changes, and then another client makes changes to Modify Table View, then field names will appear blank the next time you go into Modify Table View.
Are you in a shared environment? Does more than one user have privileges to access Modify Table View dialog?
Any other information you can provide about your computing environment could be helpful in narrowing down the possible cause.
Windows7SP1, windows updates applied (may be almost all). and FM16A v2
The file is not shared.
Another shared file was opened at the time, but no other users might not open, it is testing server only I use.
Script workspaces are opened for both file.
I can't reproduce the issue today. Sometime it takes about 3 seconds to show field names after showing only all empty checkboxes at the moment (no arrows and lock icons so different than the screen shot in first post), but finally show field names.
I tried file recovery, the log says "Recover built a new database without detecting any problems."
Then open the file and "Modify Table View", it takes about 5 seconds to show all 68 fields at first time, but only once, and show field names correctly.
I noticed that the dialog keep its size after changed, but not keep position. But now close the recovered file then open original file, the dialog begin keeping its position...
