How to create conditional formatting of Get Date function.

I am created a form which will use merge fields.  I have created several fields to get the date in the format I need.

 

My examples is as follows;

 

IncidentDateDay = Day ( Incident Date ) which will obviously return a number. 

 

I am trying to have the merge field in the layout replicate the following;

 

As reported to me this 8th day of October, 2017.

 

The text below is echo'd via the <<IncidentDayDate>> of <<IncidentMonthDate>>, <<IncidentYearDate>>.

I would like the calculation to return the day with the proper suffix/tense in relation to the date.  Meaning if 1 was the returned value , 1st would be correct.

 

Here is the result (tense) I am hoping for the days of the month

 

Suffix

st = 1, 21, 31 = 1st, 21st, 31st

nd = 2,22 = 2nd, 2nd

rd = 3, 23 = 3rd, 23rd

th for 4, 5, 6, 7, 8 ,9, 10, 11, 12, 13, 14, 15, 16, 17, 18, 19, 20, 24, 25, 26, 27, 28, 29, 30 = 4th, 5th ,6th etc.
