I need to be able to create a new record, but once created, I want to restrict any modification of the name field or address field. How do I do that?

Have one layout to change data on and another very similar where you can only search. Then you have a button to go to the other layout where you check Privilege Set to see if this user are allowed to do changes to this record
Add two global fields, one for name and one for address. Make the real name address field an auto-enter calc with the Do Not replace existing field value selection ticked.
When the record is created, the fields will be empty, when you enter text in the global fields they will populate but then you can not change them after that.
You do need to then remove the contents from the global field, but I am wanting to assume you are doing some other things around the action of new record creation
