Substituting a Nickname on a label just before printing

I have a large db of contact names, and I am developing a small application to print out name tags. Here's a current label preview:

Simple image container, and three fields First[name], Last[name], MaidenName.  I want to be able to change the large text First field to a NickName (e.g. Janet goes by "Jan") just before the label for that person is printed. And if the NickName field is then not empty (meaning someone has requested an alternative to First) then the label for that record will always use the NickName rather than First in that large text field. The underlying db starts out with no NickName entries for each record.

Can the action of clicking on the large text First name and changing it to an alternate thus create a NickName field content that will then be used for that record instead of First in the large text field?

Or, can there be a small button that the operator can click to substitute, and then enter, a NickName entry instead of the First entry?

Any good ideas on how to set this up?
