Web deployment of a survey

I have a potential project that entails putting a survey online and then producing a report in fileMaker.  The client has been using Qualex (?) to gather the data, but would like to explore how FMP could do the whole thing.  My thought is to use mySQL on their website with JavaScript to control how that part works and then ODBC/JDBC to move the data into FMP to produce the report.  As background, the number of clients that will be replying to the survey will be 150-200, over a 2 month period.  The survey includes 170-200 questions with 2000 possible answers for each client and the report is 120-150 pages long.  Any comments on this approach?  Any suggestions on how to do it better?  As I am not totally proficient in either mySQL or JavaScript, I may be asking for bids to help with that part of the system, but I won't do that until after I have the commitment from the client to proceed.
