What is the best way to make an employee evaluation checklist?

I have an existing solution that I would like to add a training section. I would like to have a list of questions that the Trainer could answer at the end of each training session. I am not sure how to set up the questions and answers. Each question would have a grade of:

  1. Unacceptable
  2. Below Standards
  3. Meets Standards
  4. Proficient
  5. Did not observe

Should I be making a static layout that lists the questions in text, then have different fields that rates the questions? Or should I have a table that stores the questions?

 

I am confused on how to proceed.

 

Here are a couple example questions:

Category 1: En Route To Scene

Question 1: Communication to dispatch

Question 2: Followed directions

Question 3: ETA reasonable

 

Category 2: On Scene

Question 1: Professionalism

Question 2: Communication to dispatch

Question 3: Scene Safety

 

Category 3: Cone Pattern

Question 1: "V" Pattern

Question 2: Lane Blockage

Question 3: Left/Right Turn Only

Question 4: "U" Turn

 

There are a total of 11 categories. Each category is rated by the five standards listed above as 1 - 5.

 

Then at the end there is a section that would allow for the trainers feedback. This would just be a stored text field.

 

Any suggestions would be greatly appreciated.
