To solve your immediate problem, define the chart to use data from current record. For the X axis, specify a calculation =

Heavens, no. A separate record for each assignment in the Assignments table, and a separate record for each grade in the Grades table. A record in the Grades table would look like this:
GradeID (auto-entered serial number)
StudentID (matchfield to the Students table)
AssignmentID (matchfield to the Assignments table)
and your relationship graph would be:
Students -< Grades >- Assignments
