tables relationship for sub-form design

I plan to create a client database.

Of which has sub form design.

The plan is that I want to keep records of each client or company, of which may have more than one factory, or more than one contact person.


For example, ABC Inc. has 2 factories. One locates in Bangkok and contact persons are Mr. Stark, Engineer, and Mr. Sam, Sales Manager.

The other lacates in Phuket and contact person is Ms. Pepper, Purchasing.


I want to design a main form page that contains details of this ABC Inc. with the sub forms of 2 addresses with 2 contact persons accordingly.


What should be the right table design and their relationships?

And how could i create these sub forms within the main form page?


Need help on this, please.
