You may need to re-think how you have the data structure set up. You likely want to have 3 tables.
1) CLIENT table with all of Bob's contact info
2) ORDERS table with each time that Bob brings his car in for service
3) LINE ITEMS table with each thing Bob wants to have done to his car
When Bob comes in the next time, you create a new record in the ORDERS table. CLIENTS are linked to ORDERS and ORDERS are linked to LINE ITEMS.
Every time you create a new ORDER the basic information is not duplicated as it resides in the CLIENT table.
This way when Bob brings in his car he can have an unlimited number of things done to his car. This is better than hard coding the service items as ITEM_1, ITEM_2, ITEM_3, etc.

Without knowing where your contact information is in your list of tables it is hard to tell you exactly.
As I wrote before - start with a table with the contact info. You'll have Bob's info in there once.
Have a second table that is for the order possibly the COMPLAINT in your structure. It should contain very little information - maybe the date and the relationship info.
When you add a new record into that table, it will not contain links to the other child tables - you'll be starting fresh with new records being created there.

For each of the fields you want cleared, there's anAuto-Enter Option named "Data". Check that box and leave it empty.
