You can have all newly defined fields added to the layout at once. Under the Edit menu option, click preferences, click layout tab, and then click add newly defined fields to current layout. When you add new fields to the table the fields will be automatically added to the layout.
Yes, I've done that.
But I want to create a layout and add all fields long after they've been created.
Enter Layout Mode.
Select New Layout
Click the object for a list view layout type
In the next window select "move all" to add all fields from a given table to the layout.
If you want to do this for an existing layout, do the same steps listed above, but then copy and paste the field objects from the new layout to the old.

You can also get much the same result with the Modify Button in a table view layout. You can shift click to select the first and last fields listed in the add fields dialog to add all fields to your table view.
