I work as a contractor from home I am trying to create a database for info when I am on a call
I have all my fields created the way I want them however I also want my reports to be created as I fill them out. I am having problems with it I can fill out the info and nothing is filling in as it should

Here is the database http://screencast.com/t/Lmmz63ktt as you can see its all text. I would like to have the capability of doing drop downs and choosing an option and having it fill it out with that value I don’t know how to do that.
Any recommendations in what I am doing wrong would be very helpful
