You will want at least 2 tables. One to track the dog information and one that is a history table.

but how can I get it to take the information from my drop drop list or edit box and put it on a excel sheet thats attached?
like I made my drop down box to include, defication, feeding, fever, dehydrated, medicated on (date)
so i have those. now when i input what happened i want it to follow over to a excel sheet that has column a date column b what i just put in the drop down field.
Well You can save your data as Excel. You can also export and then format. However, if you are trying to get your data into a pre-existing Excel file ( like a template ), it is not a trivial job.
I made my drop down list as a b c d example. So If I chose that animal did a I want it to save to an excel sheet that sayes in column a date column b ate 1 can wet food. as an example.
that way I can go into the excel sheet and say ok he hasnt had wet food in two weeks.
or just print it. am i confusing you yet :)
