The Form is in Filemaker ver???   on Windows??

Creating a Fillable PDF would be one option, but is not easy. Requires Adobe Acrobat.
There are Form creation tools on the internet, none that are free and easy.

You can make the form available using Instant Web Publishing in Filemaker.

The Department of Transporation in Washington State makes their forms downloadable into a Filemaker Runtime
Note: in Aug 2012 they upgraded from a FMPv5 runtime to a FMPv11 runtime   http://www.wsdot.wa.gov/forms/eforms.htm

You can use Save As Pdf and then email the pdf as an attachment.  Filemaker just sends plain text email unless you use attachment or a plugin.
